There are only seventeen graves that are marked with tombstones, plus one funeral home marker that give the names of those who are buried here. Most of the graves are marked with metal crosses, with an engraving of the person's initals and patient number. The markers with name and date inscriptions range between 1926 and 1947, with the exception of one burial in 1996. At this time, it is unknown to me what the date range is for the graves marked by crosses. I did not record the patient numbers during this survey, but intend to as soon as I am able. The Madison State Hospital holds a card index with the patient numbers, names, birth and death dates. For family members who want to request this information in the meantime, they should contact the MSH's Medical Records Department by calling (812) 265-7370, and request a "Release of Information" form. The family member making this request should be next of kin or a guardian of the patient. If the patient's medical records are requested, it might be helpful to know that the older records are stored in Indianapolis. The family member needs to go through the same steps, but allowing time for the MSH Staff to request the file from Indianapolis. Ruth Hoggatt - Jan. 15, 2003.
