Death registration for Indiana began in 1882, which included both county and city deaths.
(However, the Jefferson County Health Department holds a death record book for the City of Madison for 1879 - 1905, although the contents are sparse pre-1882.)
Prior to 1900, death records were filed in the county where the death occurred and may be found only at the County Health Department, not the State Health Department. After 1900, death records may be found at both the County and State Health Departments. If a death record is not found to be at the State Department, than a search at the county department should be checked and vice versa.

Requests must include the deceased's name, date of death (month and year). Fees are five dollars per copy. Personal checks will not be accepted. Please send cash or a money order. Include a SASE.
